Hospice: Clinical Manager of Support ServicesThe Clinical Manager of Support Services is primarily responsible for the day-to-day supervision of hospice support services for SW, CH, MU, MA and other supervision as assigned. The Clinical Manager of Support Services establishes, implements and evaluates goals and objectives from hospice services to meet and promote standards of quality and contribute to the total organization and philosophy.
Position: Clinical Manager of Support Services for Hospice
Salary: $100,000 - $110,000 annually, depending on experience
Service Area: Lifespark Corporate Office, St. Louis Park MN 55416. Plus, mileage reimbursement of 72.5 cents per mile when traveling to homes and/or communities applies.
Schedule: 1.0 FTE, Full - Time / Monday Friday, 8:00am-5:00pm (weekend rotation is every 4-5th and holiday and on-call rotation)

Position Functions & Responsibilities:
Coordinates and oversees all direct and indirect patient services provided by SW, CH, MU and MA personnel.
Provides guidance and counseling to TC's, SW, CH, MU and MA personnel to assist them in continually improving all aspects of hospice care services.
Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel and maintains compliance to Lifespark Hospice Standards.
Manages SW, CH, MU, MA and planning.
Provides help in assessment, planning, implementation and evaluation of patient and family caregiver care to all personnel as indicated.
Oversees and maintenance patient clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities.
Assures proper maintenance of clinical records in compliance with local, state and federal laws.
Responsible for maintenance of adequate and appropriate inventory supplies and equipment for provision of services.
Interprets operational indicators to detect census changes and increases or decreases in volume, which could impact staffing levels, revenues or expenses.
Evaluates performance of clinical team.
Develops skills and techniques in evaluating the performance of clinicians.
Hires, evaluates and terminates organization personnel.
Conduct clinical performance evaluations annually or more frequently if indicated.
Plans and implements in service and continuing education programs to meet education and training needs of the team.
Assists with the evaluation of organizational performance via performance improvement program. Productivity, quarterly and annual reviews. Assures for the quality and safe delivery of home health services provided through the organization.
Develops, implements and evaluates the orientation program for new personnel. Responsible for orientation of new team members directly or by delegating to another staff member.
Assists in organization goals, Develops, recommends and administers policies and procedures.
Stays informed about changes in the field of nursing and hospice, shares information with appropriate people.
Promotes hospice referrals in the community.
Manage safety concerns of clients and clinicians, escalating when appropriate.
Other duties as assigned
Qualifications:
LICSW
Current license to practice in MN
Minimum of 5 years' experience working with hospice patients and families
Experience in supervision of professionals and administrative team members
Experience in teaching, training, and evaluating clinical staff.
Experience in customer service.
