ElderServe Health is a mission-driven, nonprofit managed care organization serving the New York metropolitan area. ElderServe provides long-term services and supports to more than 20,000 older adults and adults with disabilities across New York City, as well as Westchester, Nassau, and Suffolk Counties. ElderServe primarily serves a frail and elderly membership who is predominantly dual-eligible and enrolled in Medicaid managed long-term care, Medicare-only and integrated Medicaid-Medicare plans. The CareSource and ElderServe affiliation addresses these critical long-term care needs to help members be as healthy and independent as possible.Job Summary:The Intake Nurse - RN is responsible for assessing potential new members for eligibility for the program and for explaining the program to them and their families. Responsible for knowing required outreach activities within the community and the assessment of eligibility and safety of potential members. Collaborates with other members of the Intake/Outreach team and the clinical team to assure the appropriate provision of services to new members. Communicates with potential members, family, referral sources and physicians to assure a seamless transition to managed long term care.Essential Functions:Conducts outreach in the community as needed.Performs the initial assessment using the UAS (Uniform Assessment System) tool.Evaluatues potential memers for eligibility and safety.Determines the care needs of potential members and formulates the initial plan of care.Communicates with the clinical team to ensure seamless delivery of service.Completes all enrollment documentation in a timely manner.Serves as a community resource for intake issues.Demonstrates the ability to integrate age specific requirements with individual developmental, mental, social and cultural needs when preforming procedures for geriatric members.Perform any other job related duties as requested.Education and Experience:Associates degree in Nursing requiredBachelor's degree preferredOne (1) year experience working in an acute care setting requiredHome care and MLTC-intake experience preferredCompetencies, Knowledge and Skills:Ability to define problems, collect data, establish facts, and draw conclusionsAbility to speak effectively before groups or customers or employees of organizationMust be able to read, write, and speak EnglishAbility to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manualsDemonstrates the ability to utilize the equipment necessary to perform their job dutiesConvey a positive attitude by being friendly, kind and interested in helping othersCreate a home-like, healing environment by being courteous and respectful to allKnowledge of Medicare and Medicaid regulationsLicensure and Certification:Current Registered Nurse license in the state of New York requiredMust have valid driver's license, vehicle and verifiable insurance. Employment in this position is conditional pending successful clearance of a driver's license record check. If the driver's license record results are unacceptable, the offer will be withdrawn or, if employee has started employment in position, employment in this position will be terminatedWorking Conditions:This is a mobile position, meaning that regular travel to different work locations, including homes, offices or other public settings, is essential.Will be exposed to weather conditions typical of the location and may be required to stand and/or sit for long periods of time.Must reside in the same territory they are assigned to work in; exceptions may be considered, due to business needMay be required to travel greater than 50% of time to perform work duties.Required to use general office equipment, such as a telephone, photocopier, fax machine, and computerFlexible hours, including possible evenings and/or weekends as needed to serve the needs of our membersRoutine travel requiredCompensation Range:$100,000 - $135,000 CareSource takes into consideration a combination of a candidate's education, training, and experience as well as the position's scope and complexity, the discretion and latitude required for the role, and other external and internal data when establishing a salary level. In addition to base compensation, you may qualify for a bonus tied to company and individual performance.
