Veteran Community Care Coordination Specialist Job description:Location: IL / RemoteReports to: Care Coordination Center ManagerSalary: $45,000-$59,000 annuallyPosition Type: Full-time Position Summary:The Veteran Community Care Coordination Specialist assists Illinois Joining Forces in providing accessible, high-quality customer support, resource navigation, and care coordination services to Illinois Service Members, Veterans, and their families. This position works within an inbound/outbound Care Coordination Center and supports clients through phone, text, chat, email, and in-person interactions by identifying needs, connecting individuals to appropriate resources, and assisting with service coordination and follow-up. This is a part-time salaried position averaging approximately 20 hours per week with a set schedule and occasional flexibility based on operational needs. Essential Responsibilities:Provide clients with information, referrals, and resource coordination through phone, text, chat, email, and face-to-face interactions.Collect and accurately document client information, demographics, and service needs in accordance with department standards.Assist clients with identifying community resources, benefits, and supportive services.Provide exceptional customer service while maintaining professionalism, empathy, and confidentiality.Respond appropriately to escalated or high-stress situations and follow internal procedures for crisis escalation and support.Maintain accurate records and update resource information as needed.Collaborate with community partners and agency stakeholders to support client needs.Conduct follow-up communication with clients as assigned to support ongoing coordination and service outcomes.Assist with special projects and organizational initiatives as assigned. Qualifications:Associate degree preferred in communications, human services, business, public administration, social services, or related field. Equivalent work experience may be considered in lieu of degree.Customer service, call center, community resource, or Veteran-support experience preferred.Knowledge of military culture and Veteran resources preferred.Ability to communicate professionally and compassionately with diverse populations.Experience using Microsoft Office and web-based systems preferred.Training related to trauma-informed communication, crisis response, or suicide prevention preferred but not required.We strongly encourage qualified Veterans to apply.
