In this role, you will collaborate with an interdisciplinary team to deliver compassionate, high-quality hospice care while managing patient plans and documentation with excellence.Required QualificationsAssociate degree or higher in NursingActive and unrestricted Registered Nurse (RN) license in the State of PennsylvaniaMinimum of 2 years of Hospice nursing experienceAt least 1 year of experience using Electronic Medical Records (EMR)Experience with TruChart EHR or other EMR systems is acceptableValid Drivers License, reliable transportation, and current auto insuranceAbility and willingness to travel locallyComfort using tablets, mobile devices, and clinical software Preferred QualificationsRN license number provided at time of application Key ResponsibilitiesCollaborate with the interdisciplinary team to manage and deliver individualized patient care plansProvide hospice nursing care to patients throughout Pittsburgh and surrounding areasEnsure timely, accurate charting and documentation in compliance with care standardsEffectively manage time, scheduling, and patient visits independentlyUtilize EMR systems and mobile technology to support efficient care delivery Schedule & TravelFull-time | Monday-Friday | 8:00 AM - 5:00 PMShared on-call coverage for holidays (approximately two holidays per year)Local travel requiredWhy Apply?
