As part of our HouseCalls team, you’ll provide preventive, in-home health assessments—helping patients stay well while connecting them to the care and resources they need.This is a non-prescribing, field-based role focused on improving health outcomes through education, gap closure, and collaboration with primary care providers.What You’ll DoConduct in-home health assessments (history, meds, vitals, physical exam)Perform screenings and point-of-care testing (as appropriate)Identify and document conditions for care planningCommunicate findings to PCPs to close care gapsEducate patients on conditions, medications, and next stepsAddress social needs and connect patients to resourcesRecognize urgent issues and escalate as neededRequired QualificationsNP: ANCC/AANP certification (Family, Adult, Geri, or AGPCNP)PA: NCCPA certificationActive, unrestricted state license (or ability to obtain)Valid driver’s license + reliable transportationAbility to perform in-home visits (including mobility and lifting up to 30 lbs)Preferred1+ year clinical experience (primary care, geriatrics, or home health)Comfort with EMR/technologyExperience working with Medicare/geriatric populationsWhy Join OptumStrong support and autonomy in the fieldClear expectations and growth opportunitiesMission-driven work improving patient access and outcomesAt UnitedHealth Group, our mission is to help people live healthier lives and make the health system work better for everyone.
