Nurse Practitioner | Geriatrics Location: Brooklyn, NY Employer: AAS Locum Tenens Pay: Competitive weekly pay (inquire for details) Start Date: ASAP About the Position We are seeking an experienced Spanish-Speaking Physician Assistant for a Locums assignment with potential to convert to Perm at a primary care practice serving geriatric patients in a value-based care model. This is a hybrid role with blend of coverage in outpatient clinics as well as home-based patient care. Compensation: $75 – $90 per hour Position Details: Schedule: Full Time Hours Guaranteed Monday – Friday - 5 Days Per Week Patient Load: LOW DAILY CENSUS Home Care: 3–5 patients per day Clinic: Small census of 7–10 patients per day Setting: Hybrid (Outpatient clinic home-based care), value-based primary care for elderly patients Responsibilities: Deliver comprehensive primary care services, including preventive care, chronic disease management, and acute care to Geriatric patients Diagnose, treat, prescribe medications, and develop individualized treatment plans Order and interpret diagnostic tests (labs, imaging, etc.) Document accurately in the EHR system Provide patient education and promote wellness initiatives Collaborate with physicians, nurses, and support staff to ensure coordinated, high-quality care Refer patients to specialists and manage follow-ups Ensure compliance with state, federal, and professional regulations Requirements: Active PA license in NY Minimum 2 years of primary care experience National Board Certification Spanish Speaking Medicaid and Medicare Provider Id required
