Our Mission is to provide top quality professional nutrition services devoted to health and wellness with a commitment to exceptional customer satisfaction, utilizing appropriate resources, with coordination across the continuum of care.Role DescriptionThis is a part time or full-time role for a Consultant Dietitian in Long term Care/skilled nursing rehab based in Wytheville, Va with flexibility for remote work. Relocation assistance will be provided to the right candidate.The RD will be responsible for providing medical nutrition therapy, managing weights and monitoring skin interventions, enteral feeding, monthly dialysis reviews, and improving overall nutrition for clients in skilled nursing/rehab.Conduct nutritional assessments for new admits, significant weight changes, pressure injuries, dialysis, and calculate enteral nutrition/TPN for residents in long-term careProvide dietary services following state and federal laws and regulations, and corporate guidelines.Clinical Charting (if required in states or corporations) weights, new admits, high-risk chartingNutrition education and in-service training for staff development and regulatory complianceImplementation of the plan of correctionsHelp prepare community for state surveyQualificationsMust maintain active membership and registration with the Commission on Dietetic RegistrationStrong communication and interpersonal skillsAbility to work autonomously and collaborate with a teamExperience in geriatrics, renal, oncology, or food safety and sanitation is a plus but not requiredState licensure in Connecticut or able to obtain prior to start dateIndustryLong term Care, LTAC, skilled nursing, rehab
