We serve from a place of favor, and we never forget the sacredness of what we do.A Calling with ImpactAt Lifted Hospice, our Social Workers play a vital role-providing emotional support, counseling, and practical guidance when patients and families are navigating one of life's most difficult seasons.As a Social Worker, you'll assess each patient's psychosocial needs and deliver care that addresses emotional, financial, and family dynamics. You'll help patients and caregivers understand their options, cope with change, and access the resources they need for comfort and stability. You'll also serve as a key part of the interdisciplinary team-bringing insight, advocacy, and compassion to the plan of care.Your Role in the MissionAs a Social Worker at Lifted Hospice, you are a steady and compassionate guide through times of uncertainty. You bring calm to chaos, clarity to confusion, and strength to families facing what is often the hardest season of their lives.You listen without judgment, speak with purpose, and help people feel supported not just in what they're facing, but in who they are. Your presence reminds every patient and caregiver that they are not alone, and that even in difficult moments, dignity, peace, and connection are possible.Key Responsibilities Include:Assessing needs - Completing timely psychosocial assessments to understand the emotional, social, financial, and environmental needs of each patient and family.Providing counseling - Offering support around end-of-life issues, grief, family dynamics, and care transitions with compassion and clarity.Helping with planning - Assisting patients and families with advance directives, funeral planning, discharge, and changes in level of care.Working as one team - Collaborating with the IDT to build care plans that support the whole person and reflect patient and family goals.Connecting to resources - Helping patients access benefits, community programs, and practical support to reduce stress and improve quality of life.Documenting clearly - Recording assessments, updates, and interventions accurately and on time.Protecting dignity - Promoting patient privacy, respect, and comfort in every interaction.Continuing to grow - Participating in team meetings, required education, and quality improvement efforts to serve with excellence.Skills and Qualifications:Master's degree in Social Work (MSW) from a CSWE accredited program; or Bachelor's degree in Social Work (BSW) or related field (psychology, sociology, etc.) with qualifying experience and MSW supervision as required by regulation.Licensed in Texas as an LMSW or LBSW, in good standing with the Texas State Board of Social Worker Examiners.Experience in hospice or palliative care (1+ year preferred) and a strong understanding of end-of-life psychosocial needs.Knowledge of local community resources and ability to connect patients and families with support.Strong interpersonal and communication skills, including the ability to work with diverse personalities and cultures.Comfortable with technology-able to document care and communicate using EMR systems and mobile tools.Valid Texas driver's license, reliable transportation, and current auto liability insurance required for travel throughout the service area.Where You'll WorkIn this role, you'll spend your time serving patients and families wherever they call home-whether in a private residence, long-term care facility, or nursing facility. Your work will take place in varied environments, both indoors and outdoors, as you meet people where they are in some of life's most vulnerable moments.
