Joseph's House & Shelter is currently seeing a Diversion Program Advocate. The right candidate would be able to:
Carry a caseload of clients diverted from Shelter stay.
Provide direct case management with the primary goal of assisting clients in obtaining appropriate, sustainable housing.
Provide direct advocacy services to clients receiving diversion services in a non-judgmental and respectful manner.
Other key duties for this position involve ability to do the following:
Work within a best practice framework of Harm Reduction, Motivational Interviewing and Trauma-Informed Care.
When possible, provide support toward family reunification/mediation and mitigating barriers to securing alternative options for safe housing.
Provide engagement, intake procedures, case management, and supportive services to persons eligible for the diversion program administered by Joseph's House and Shelter.
Facilitate case management with other service agencies to ensure that the client has access to appropriate services;
When possible, provide transportation for clients to community services and housing related appointments.
Administrative duties may include, but are not limited to:
Enter and maintain client information and case notes in the HMIS database and follow through with assigned tasks.
Keep organized and have contemporaneous documentation.
Assist the Diversion Program Manager as needed/requested, including tasks not specifically outlined in this description.
Attend and participate in supervisory meetings on a regular basis.
Attend and participate in regularly scheduled staff meetings, and attend team meetings.
Participate in staff training as requested or required.
Clean and valid Driver's License is required.
Job Type: Full-time
