Psychiatric - Mental Health Nurse Practitioner
Job description: ESSENTIAL DUTIES AND RESPONSIBILITIES:Assess and treat adults experiencing symptoms of a behavioral health disorder according to DMS 5.Prescribe, recommend, or discontinue drugs and treatment to treat mental, emotional, or behavioral disorders in accordance with the Collaborative Practice Agreement and the states Nurse Practice Act.Experience in starting and managing psychiatric medications.Deliver patient care based on obligations set forth by the State of South Carolina permissible under their license.First line Psych provider at the SNF facility and represents PSG at the facility.Evaluate and treat new admissions, follow-ups, and long-term care patients -based on PSG guidelines.Counsel patients using advanced psychotherapeutic techniques.Liaison with primary care team and specialist to help coordinate the continuum of careUpdate family members on patient status and med changesWork directly with facility to maintain compliance with appropriate paperwork for Anti-psychotic med adherence and initiating Gradual Dose reductions.Work directly with facility supervising MD and team to deliver quality careRepresent PSG in a professional business manor.Solve acute Psychiatric problems for PSG patients and Management at their facilities.Responsible for daily documentation within the EMRResponsible for daily E/M billing based on guidelines/expectations/opportunities set forth by PSG guidelines for coding.Work in a professional, approachable, and cordial manorCreate a partnership between care coordination/oversight and care delivery at the facility.Participate in improvement initiatives set forth by PSGWork directly with MA to help organize and deliver quality care set forth by PSG guidelines.Maintain appropriate census set forth by PSG guidelines for productivity and Quality outcomes.QUALIFICATIONS: Graduate of approved nursing program ANCC or AANP with Psych experience or PMHNP SC license required. Registered Nurse certification Perform duties within a schedule from 8-4pm M-F CME should be up to date Passion to deliver quality care and to help make a difference in the post-acute setting. Must have a strong clinical background and experience in Psych and well versed in Must have strong interpersonal skills, be a team player, and the ability to work independently. Advanced medical knowledge of common chronic disease states including psychosis, delirium and anxiety. Must be able to recognize and solve acute problems and treat them appropriately.
