Care Coordinator

Department: Mozaic at HomeSupervisor: VP Community ServicesShift: Monday-Friday 40 hours per weekJob ObjectiveThe Care Coordinator is responsible for developing, implementing, and managing individualized care plans for Members through effective, person-centered case management. This role ensures coordination of services, promotes quality outcomes, values strong and trusting relationships with Members and their families, and supports Members in maintaining independence and well-being within the community.Minimum QualificationsEducation: Registered Nurse (RN), Licensed Practical Nurse, (LPN), Social Worker, or equivalent combination of education and relevant experienceExperience: Minimum of 2-3 years working with older adults in a healthcare setting; home care experience preferredLicensure/Certification: Valid Connecticut Driver's LicenseKnowledge/Skills/AbilitiesStrong organizational skills with the ability to manage multiple priorities and deadlinesExcellent verbal and written communication skillsProficiency in Microsoft Office and care management systemsKnowledge of community-based services and resources for older adultsStrong assessment, problem-solving, and critical thinking skillsAbility to work both independently and collaborativelyEssential Job FunctionsConduct comprehensive pre-admission assessments including cognitive, psychosocial, nutritional, and environmental evaluationsManage assigned caseload with ongoing communication with Members and familiesDevelop, implement, and update individualized care plansMaintain accurate documentation in Member records and CRM systemCoordinate and oversee service deliverySupport wellness initiativesUnderstand the broad financials of the Mozaic at Home program, including each Member's daily benefit according to their selected plan option, the cost of care, and how they work together.Perform additional duties as assignedPhysical And Mental RequirementsAbility to sit or stand for long periods of time.Ability to speak clearly and be understood.Able to hear sounds within some background of noise as in answering the phone, intercom, etc.Visual acuity to see computers and read printed documents.Ability to use company software and equipment to accomplish electronic tasks.Ability to concentrate on fine details with interruptions.Ability to attend to tasks/functions for more than 60 minutes at a time.Ability to understand and relate to the reasons behind several related concepts.Ability to remember multiple tasks /assignments given to self and others over long periods of time.Ability to perform gross body coordination such as walking, stooping, filing, etc.Ability to perform tasks which require hand eye coordination such as keyboard skills, writing or picking up paper.Environmental ConditionsRare exposure to blood, body tissue or fluids.Possible exposure to high/low temperatures.This job description is not designed to cover or contain every possible listing of activities, duties or responsibilities that are required of the employee for this job.EOE M/F/D/VSalary: $27.50 - $33.69 per hour
