This opportunity is ideal for a physician who enjoys building lasting patient relationships, working in a collaborative care model, and practicing medicine with a strong focus on quality, prevention, and continuity of care.This is a full-time outpatient primary care position serving an adult patient population. The practice emphasizes chronic disease management, preventive care, medication management, wellness visits, and long-term patient engagement. Providers are supported by a team-based clinical structure designed to help deliver efficient, thoughtful, patient-centered care.
