Advanced Practice Provider (NP/PA) - COPC - Westerville Internal Medicine and Endocrinology

Central Ohio Primary Care is seeking a full-time Advanced Practice Provider (NP/PA-C) for Westerville Internal Medicine, 625 Africa Rd., Suite 200, Westerville, Ohio 43082.The APP, in collaboration with the Endocrinologist, coordinates patient care. This position monitors the patient’s status and collects data to evaluate and improve care by providing a comprehensive assessment, implementing the appropriate treatment, and evaluating the patient’s progress.About the practice:This COPC practice has established a reputation as a primary care group that serves the community. They provide the best primary care for all ages with a focus on health education, disease prevention, and management to keep their patients well.It is a large outpatient practice consisting of 5 Internal medicine physicians and an Endocrinologist. There is an Office Manager, an APP, several medical assistants, patient service reps, an RN, an LPN, a Clinical Lead, Clinical Assts, and a phlebotomist to support the providers and patients.Hours are: Full Time: Monday, Wednesday, Thursday, Friday 8:00 am-4:30 pm, Tuesday 8:00 am- 12 pm.About the role:This APP role will work with the Endocrinologist at the practice. Experience in an endocrine/diabetes setting would be given preference. New graduates will be accepted as potential candidates. Training and onboarding are provided.Key Responsibilities:Conduct thorough assessments, including medical history reviews and physical examinations, to diagnose and treat acute and chronic health conditions in patients. Provide a full spectrum of services, including seeing new patients, managing follow-up visits, health education, disease prevention, and stepping in when the Endocrinologist is absent.Develop individualized treatment plans for patients with chronic diseases such as diabetes, thyroid, and other endocrine conditions. Monitor disease progression, adjust treatment regimens, medication management, and educate patients on self-management strategies.Empower patients and families by providing comprehensive education on health maintenance, disease prevention, and treatment options. Address questions, concerns, and health-related inquiries to promote informed decision-making and adherence to care plans.Collaborate with the Endocrinologist and nurses daily to coordinate patient care as needed.Maintain accurate and timely documentation of patient encounters, treatment plans, and follow-up care using electronic health record systems in compliance with regulatory standards and organizational policies. COPC uses EPIC.Qualifications:Master’s degree in nursing or physician studies from an accredited program.Current licensure as a Nurse Practitioner or Physician Assistant for the state of OhioNational certification as a Family Nurse Practitioner (FNP) or Physician Assistant (PA).A minimum of 1 year of practical working experience as an NP/PA post-graduation in an outpatient practice setting is preferred.Strong clinical assessment, diagnostic, and treatment skills.Excellent communication and interpersonal skills.Ability to work independently and collaboratively within a multidisciplinary team.EPIC EHR experience is helpful.
