RN and LVN

NURSE RN and LVN Established and growing home health agency for non-medical home health care. We has immediate opening for RN, LVN, PAS and PHC program in Dallas, Fort Worth area. JOB Responsibilities: RN assess the patient health problems and develop and implement nursing care plans as ordered by the physician. Evaluation and Admission are done at the patient residence. Complete the OASIS, and other admission documentation. Conduct SN visit, LVN supervisory visit, HHA supervisory visit, and work with other disciplines like therapist. Manage Medicare, Medicaid, and other Insurance based on clients and cases. Participate in QAPI, infection control, work to attain outstanding out come and Five Star rating. LVN do the SN visits under the supervision of the RN. Provide basic patient care or treatments, such as wound care, injections, PT&INR, teaching the client on how to heal and or manage various diagnosis. Work with therapist and HHA as delegated to your client. Job Requirements: Must be currently licensed as a Registered Nurse in the State of Texas. Prefer 2 plus years in the home health field with work experience proving care to the clients. Experience in home health office doing similar job may be substituted for experience.
