The MA also provides direct support to the Upperline Plus Primary Care Provider to ensure smooth clinical operations and effective team-based care.The ideal candidate is organized, patient-focused, and comfortable working at the top of their scope in a team-based care model. As Upperline continues to expand, this individual will be able to explore a wide range of career opportunities within the company.Most importantly, candidates must demonstrate compassion for patient needs, have great attention to detail and thrive in a collaborative environment.This position will be on-site in our Acworth and Cartersville offices.Primary Care Medical Assistant ResponsibilitiesRoom patients, obtain and document vital signs, medical history, and chief complaintsAssist NP with health assessments and in clinic visits with Upperline Plus patients being seen in our clinicsFacilitate and support virtual provider encounters from the clinic settingAccurately document clinical information in the electronic health record (EHR)Performs thorough and accurate medication reconciliation at each patient encounter by reviewing, verifying, and consolidating all prescription medications, over-the-counter drugs, supplements, and herbal productsIdentify medication discrepancies or potential concerns; and ensures the reconciled medication list is complete, current, and accurately documented in the electronic medical record for provider reviewTelephonic outreach and engagement with Upperline Plus patients to coordinate ongoing preventative careManage in-basket messages, prescription refill requests, and prior authorizationsCare coordination activities as assigned including, but not limited to, medical records requests, order management, appointment scheduling, managing patient care referrals, DME referrals, etc.Identifying community resources and assisting Upperline's patients and families in accessing those resourcesPrepare and maintain equipment and supplies commonly used by the Upperline Plus providerCollaborate with providers, nurses, care managers, and social services to address medical and social determinants of healthServe as the Upperline Plus provider's primary point of support for workflow optimization and patient flow managementIdentify barriers to care (transportation, medication access, health literacy) and escalate appropriatelyParticipation in interdisciplinary team meetings & daily clinic team huddlesPerform clinical tasks within scope (injections, phlebotomy, point-of-care testing)Other related duties as deemed necessary by Upperline HealthPrimary Care Medical Assistant QualificationsCertified MA or minimum of 2 years' experience in a medical primary care settingExperience in a primary care /outpatient clinic setting is strongly preferredCertified MA or phlebotomy certification strongly preferredDemonstrates strong knowledge of medical terminology, disease processes, and basic pharmacology, including medication classifications, common indications, dosing ranges, and potential side effectsAbility to accurately interpret provider documentation, diagnostic orders, and treatment plansClinical care coordination or care management experience is idealPrevious experience assisting or providing care to the adult geriatric patient population is preferredPrior wound care and foot care experience is a plusBilingual English/Spanish is helpfulAbility to work collaboratively in a team-based care modelPrevious experience working in an EMR system ideally, AthenaHealthProficiency using Word, Excel and other Microsoft Office products is necessaryMust have reliable transportation, valid driver's license and minimum state required liability auto insuranceExtremely strong customer-service orientation and work ethicComfort managing multiple tasks and prioritizing in a fast-paced environmentEmpathy, professionalism, and cultural sensitivitySelf-starter with an entrepreneurial spirit and demonstrated problem solving skillsExcellent oral and written communication skills to ensure that each of your interactions with patients, family caregivers, clinicians, and team members is effective and meaningfulCompensationCompensation is commensurate to compensation for similar positions in the region and based on prior training and experience.Job Type: Full-timeEqual Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.
