Founding Care Manager (Hybrid Remote + In-Home Senior Support)

We help seniors remain independent, safe, and connected at home through proactive care management, chronic care coordination, remote monitoring, and in-home support.We are looking for a high-performing, emotionally intelligent Care Manager to become one of the foundational members of our team.
