The Social Worker will provide emotional support, counseling, and assistance to patients and their families, connecting them with resources and services that address their social, emotional, and healthcare needs. Responsibilities include conducting assessments, developing care plans, and collaborating with interdisciplinary teams to advocate for patient welfare. Additional duties may involve crisis intervention and facilitating communication between patients and healthcare professionals.
