SHARP (small high acuity/risk panel) is a Nurse Practitioner led small panel primary care program. Johns Hopkins SHARP providers co-manage a small panel of high-risk adult patients in a primary care practice, enhancing standard PCP care. The SHARP Nurse Practitioner collaborates with 3-5 established PCPs to provide enhanced, high touch, independent and collaborative primary care to a select group of high-risk patients. SHARP NP enjoy extended visits and significantly lower visit volumes than seen in standard primary care practices.
