LPN - Continuing Care• Skilled Nursing Facility• 625 Bethany Rd, Albemarle, NC 28001• Radius rule: Traveler's permanent address must be 50 miles outside of home unit• Department Information:• Beds: 90• Minimum Years of Experience required? 18 months in SNF/LTC Rehab facility• Will you accept a first-time traveler? Not preferred• Patient Types/Common Diagnoses:• Short-term and Long-term services offered to patients recovering from an illness, injury or accident• Stroke, Amputation, Neurological Disorders, s/p Joint Replacement, etc• No Memory Care• Patient Ratios:• RN: 1:18-20 Days; 1:26 Nights• LPN: 1:18-20 Days; 1:26 Nights• CNA: 1:13 Days; 1:15 Nights• Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements• RN: BLS• LPN: BLS• CNA: BLS• Skills required:• Previous Long-Term Care exp to include: Trach management (refresher provided during orientation), foley care, wound experience, dressing changes, IV starts/management, tube feedings• Support within the Department:• Nursing Leadership• Admin Nurse will do any admission - admissions will be approx. 1-2 per day• HCT• Therapies• Case Management• Technology/Equipment:• EMR: EPIC• IV Pump: Alaris• Med dispensing: Omnicell• Floating:• Within Continuing Care and scope of practice• Will be required to float per region needs - will require floating to all facilities listed below, as needed, within scope of practice - DECLINE if not agreeable• Rehab Region - Cleveland Pines, Huntersville Oaks, Jesse Helms, Sardis Oaks, and Stanly Manor• If floating mid-shift, Traveler will be paid for the time traveling between facilities (will remain on the clock)• Orientation (facility and unit):• 2 days departmental• Other notes:• Scrub Color: LPN - Ceil Blue• Parking: Free
