Home-Based Caseworker

Job Summary We are seeking a dedicated and compassionate Case Worker to join our team. The ideal candidate will play a vital role in supporting individuals and families in navigating complex social systems. This position requires a strong commitment to advocacy, crisis intervention, and the ability to work collaboratively with diverse populations, including individuals with disabilities and children in need of protective services. Duties Conduct assessments to determine the needs of clients and develop personalized service plans. Utilize motivational interviewing techniques to empower clients in setting and achieving their goals. Provide crisis intervention support to individuals and families facing immediate challenges. Collaborate with community resources, including healthcare providers, educational institutions, and social service agencies. Manage conflict effectively while maintaining a supportive environment for clients. Work closely with children and families involved in child protective services, ensuring their safety and well-being. Advocate for clients’ rights within the child welfare system and assist them in accessing necessary resources. Maintain accurate documentation of client interactions, progress notes, and case management activities. Stay informed about family law regulations and best practices in social work and behavioral health. Experience A background in social work or related fields is required 5 years Experience working with children, particularly in child welfare settings. Familiarity with crisis management techniques and behavioral health principles. Strong skills in conflict management and the ability to navigate sensitive situations with empathy.
