SummaryThis position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Post Acute Rehabilitation - Polytrauma and Long Term Care demonstrates proficiency in decision-making and using the nursing process for complex situations that effect performance outcomes at the point of care through collaboration strategies with others to provide comprehensive nursing care to resident/residents assigned to the assigned CLC neighborhood where conditions are usually of a non-critical natureMajor duties include - but are not limited to: Performing initial history and assessment - care planning to include but not limited to discharge planning - pain management - nutrition - Bar Code Medication Administration (BCMA) - and the Computerized Resident Record System (CPRS)and/or other electronic health record documentationResident teaching and care - tube feeding - Electrocardiogram - use of ceiling lifts - care of the residents with tracheostomies - glucometer management - wound care and management and pulse oximetryParticipates in educational activities to improve and maintain clinical knowledge and skills and enhance role performanceAssumes responsibility/accountability for evaluation of all delegated tasks and is self-directed in the identification of learning needs and development of individual performance plansInitiates and utilizes positive -
