Social Worker Responsibilities: Assists with resident and family transition into the facilityCompletes a comprehensive Psychosocial Assessment for each resident that identifies social, emotional, and psychological needs and strengthsIdentifies medical related social needs of residents, and provides appropriate services to meet the individualCollects and compiles resident social information into personal file upon admissionMaintains lists of facility and community resources available to the residents that can provide assistance in Social Services and clinical carePlans, organizes and implements Social Service programs in the facilityEvaluates effectiveness of Social Service programs in the facility and collaborates the Social Services team, Administration and other facility staff to offer best practices to the residentsAssesses each resident for discharge and coordinates discharges in the facilityAdvocates for resident’s best interest and total care in Social Services and clinical careConducts resident interviews and ensures that relevant MDS sections (i.e. cognitive, mood, behavior, patient goal setting) and Care Area Assessments are completed in accordance with regulationPerforms other related job duties as requiredAny other tasks assigned by the Executive Director. Qualifications: Master's or Bachelor’s DegreeLong Term Care experienceAbility to handle numerous responsibilities at the same time.Must have excellent communication and organizational skills.
