MDS RN or LPN Position

Ark healthcare & Rehabilitation at St. Camillus Center - Ark Healthcare and Rehabilitation at Saint Camillus, located in the heart of Stamford, CT seeks a Full-Time, Monday thru Friday, Full-Time MDS RN or LPN Coordinator.This EXCITING opportunity will afford fully customized training to the right candidate willing to learn this role.Primary responsibilities.Contributes to the facility interdisciplinary RAI process while following RAI guidelines for completion, computer coding and electronic transmissions of the MDSCoordinates the completion of the resident assessment instrument: MDS, CATs, and CAAs, in accordance with current rules, regulations, and guidelines that govern the resident assessmentManages the overall process and tracking of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for services provided within the Center.Conduct concurrent MDS reviews to assure achievement of maximum allowable PDPM categories.Attends/participates in facility IDT meetings as directedIdentifies changes in condition in conjunction with IDT and coordinates scheduling and completion of indicated assessmentsIntegrate information from nursing, dietary, social services, restorative, rehabilitation, and physician services to ensure appropriate reimbursement.Candidate will possess:Unencumbered State of CT RN or LPN LicenseExperience with Point Click Care Preferred1 year in Long Term CareGreat attention to detail and great organizational skillsArk Healthcare and Rehabilitation is an EOE
