Job Overview
We are seeking a dedicated and experienced Registered Nurse who specializes in Home Health for a full time position to oversee the delivery of high-quality patient care in a home health setting. The ideal candidate will possess strong clinical skills, leadership abilities, and a passion for providing compassionate care to patients in their homes. This role requires a comprehensive understanding of home health regulations and standards, as well as the ability to manage a team of healthcare professionals effectively.
Duties
Supervise and coordinate home health services, ensuring compliance with state and federal regulations.
Conduct patient assessments and develop individualized care plans based on clinical needs.
Oversee nursing staff and ensure adherence to best practices in patient care.
Manage patient documentation and maintain accurate medical records in compliance with HIPAA regulations.
Collaborate with interdisciplinary teams to provide comprehensive care, including case management and discharge planning.
Provide education and support to patients and families regarding health conditions, medications, and treatment plans.
Facilitate effective communication between healthcare providers, patients, and families.
Ensure quality assurance through regular audits of patient care services and adherence to NCQA standards
Maintain knowledge of current trends in home health care, including infection control protocols and emergency preparedness.

Qualifications
Current Registered Nurse (RN) license in the state of practice.
Proven experience in home health care or related fields such as long-term care, assisted living, or hospice & palliative medicine.
Familiarity with EMR/EHR systems such as WellSky is preferred.
Strong clinical skills including IV infusion, medication administration, patient assessment, wound care, and basic life support.
Experience with diverse patient populations including geriatrics, pediatrics, or individuals with developmental disabilities is a plus.
Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), CPT coding, and documentation practices.
Critical thinking skills with the ability to make sound decisions under pressure; experience in acute care settings such as ICU or emergency medicine is advantageous.
Excellent communication skills with the ability to educate patients effectively on their health conditions and treatment options.
