New Paltz Center is hiring an MDS Coordinator in New Paltz, NY.DUTIES:Completing accurate assessments, MDS & care plans as assignedInitiating care plans and supporting activities as assignedCreating and distributing monthly care plan calendars in a timely fashionMaintaining & updating all care plans and assessments as requiredMonitoring & auditing clinical records, ensuring accuracy & timelinessInforming DON of persistent issues related to non-compliant documentationProtecting the confidentiality of Resident & Facility information at all timesREQUIREMENTS:MUST HAVE PRIOR MDS 3.0 EXPERIENCEValid New York RN LicenseLong Term Care Experience Required!Must be highly organized, professional & motivatedShould have solid computer skillsExcellent communication skillsShould be friendly and a team workerNew Paltz Center for Rehabilitation and Nursing is a 77-bed rehabilitation and skilled nursing facility located in a quiet woodsy neighborhood outside of town.
