They are a community-based hospital and offer Cancer Care, multiple Cath labs, most specialties and are #1 goal is to keep our patients at home for their health care needs!/ph3Ideal Candidate:/h3ulliCurrent Colorado pharmacist license or eligibility to obtain/lili2-5 years pharmacist experience in a hospital setting/liliExperience with CODE BLUE response/liliExperience with medication reconciliation and drug therapy management/liliWilling to work night shift if required/liliWanting to work for a rural, community based hospital/li/ulh3General Objectives:/h3ulliPrepares and dispenses medication orders per physician request./liliMaintains medication stock in patient care areas./liliEnsures safe, appropriate, cost-effective medication therapies for patients according to policies, procedures and protocols./li/ulh3Primary Duties and Responsibilities:/h3ulliInterprets medication orders; accurately transcribes to, and maintains computerized patient medication profiles./liliAccurately compounds and dispenses pharmaceuticals, including IV admixture preparations, chemotherapy, and parenteral nutrition products./liliComplies with all applicable federal, state and local laws and regulations. Issues controlled substances to patient care area and maintains records as required by law./liliMonitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose./liliPerforms pharmokinetic drug dosing per hospital protocol./liliReviews/interprets CS data for antibiotic appropriateness, recommending changes as needed./liliAccurately reads, extracts, and interprets patient chart information./liliAccurately detects and reports adverse drug reactions./liliSustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection./liliProvides clinical consultation and clarification to practitioners.
