Per Diem Nurse Practitioner - Pediatric Community-Based Care (HCBS)

Location: 5 Boroughs of NYCEmployment Type: Flexible, Per DiemCompensation: $170/hourWe are seeking a compassionate and experienced Nurse Practitioner (NP) to provide high-quality care for medically fragile patients under the Home and Community-Based Services (HCBS) program. This role focuses on delivering patient-centered care, including pain and symptom management, with an emphasis on improving quality of life.Key Responsibilities:Provide comprehensive assessments and ongoing care for medically fragile patientsDeliver pain and symptom management services in home/community settingsCollaborate with interdisciplinary teams to develop and implement care plansSupport patients and families, including those requiring end-of-life careEnsure compliance with all HCBS and New York State regulationsRequirements: Active Nurse Practitioner license in New YorkCertification or qualification in Pain and Symptom ManagementMinimum 3 years of experience working with medically fragile populationsAt least 1 year of clinical experience with pediatric patientsMinimum of 3,600 clinical hours as a Nurse PractitionerExperience involving end-of-life care strongly preferredStrong clinical, communication, and documentation skillsWhy Join Us?
