We are seeking a Case Management Coordinator who thrives in a structured yet fast‑paced environment and is passionate about helping clients navigate the recovery process.
Position Summary
The Case Management Coordinator oversees and supports all case management functions within the facility. This role ensures clients receive timely, appropriate, and coordinated services across internal programs and external community resources. The Coordinator works closely with clinical, administrative, and leadership teams to maintain compliance, streamline processes, and enhance the client experience.
Key Responsibilities
Coordinate and oversee daily case management operations, ensuring consistent, high‑quality service delivery.
Serve as the primary point of contact for client referrals, assessments, and service coordination.
Support clients in accessing community resources, benefits, housing, transportation, and aftercare services.
Maintain accurate, timely documentation in accordance with state, federal, and facility standards.
Collaborate with clinical staff to support individualized treatment planning and continuity of care.
Track caseloads, service timelines, and follow‑up requirements to ensure compliance.
Assist with developing and refining case management policies, workflows, and best practices.
Participate in interdisciplinary team meetings and contribute to program improvement initiatives.
Provide guidance and support to case management staff as needed.
Qualifications
Experience in behavioral health, substance use disorder treatment, social services, or a related field.
Strong understanding of community resources, benefits systems, and recovery‑oriented services.
Excellent communication, organization, and problem‑solving skills.
Ability to maintain professionalism, confidentiality, and empathy in all client interactions.
Proficiency with electronic health records and documentation standards.
Ability to work collaboratively within a multidisciplinary team.
Preferred Qualifications
Associate’s or Bachelor’s degree in Human Services, Social Work, Psychology, or related field.
Prior experience in case management or care coordination.
Familiarity with Pennsylvania SUD treatment regulations.
