Completes all documentation in a timely and accurate manner. Participates in clinical record reviews, agency staff meetings, therapy meetings, and other activities as requested. Provides supervision and guidance to aides, ensuring compliance with care plans and documentation requirements. Performs evaluations and utilizes findings to determine diagnosis (for SLP services), prognosis, and plan of care in consultation with the physician. Determines specific interventions, duration, and frequency needed to achieve anticipated goals and outcomes. Observes and records activities and findings in the clinical record, reporting patient reactions and changes to the physician or primary Case Manager. Assesses patient responses to interventions and performs reassessments as required. Instructs other health team personnel in phases of speech therapy relevant to patient care. Instructs patients and caregivers on speech language improvement programs, including adaptive equipment and home programs. Selects, applies, or modifies skilled interventions to achieve the best patient outcomes.
