In this role, you will be responsible for managing patient care in a home health setting, ensuring that patients receive high-quality care tailored to their individual needs. Responsibilities:Coordinate and manage patient care plans in home health, providing comprehensive support to patients and their families. Conduct thorough assessments and evaluations of patients' health status and needs. Collaborate with healthcare professionals to develop and implement effective care strategies. Educate patients and their families about healthcare management and available resources. Ensure compliance with all relevant regulations and standards for home health care. Travel to patients' homes as required; mileage reimbursement will be provided at the government rate. Qualifications:Current Registered Nurse (RN) license in the state of California. Home health experience is preferred. Must possess a clean driving record.
