If interested in this Home Health LPN position APPLY NOW for IMMEDIATE consideration Brittney Blackman | bblackman@alinestaffing.com | text 5867107970 HOME HEALTH LPN | DETAILS AND COMPENSATION: Location: Virginia Beach, VA 23452 – 100% on-site/home health settings Covering: Chesapeake, Virginia Beach, Portsmouth, Suffolk, and Norfolk Payrate: $37.30/hr + $0.54/mile Required Availability: Full-Time | Monday – Friday, 8:00 AM – 5:00 PM On-Call/OT: Generally not required, but must be available if necessary. HOME HEALTH LPN | SUMMARY AND HIGHLIGHTS: The Home Health LPN will provide direct clinical care and monitoring—including medication administration, wound care, and vital sign assessment—to patients within their community while ensuring strict adherence to infection control and physician-led care plans. This role focuses on maintaining patient dignity and independence by coordinating with interdisciplinary therapists and accurately documenting all medical outcomes. This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates HOME HEALTH LPN | RESPONSIBILITIES: Direct Patient Care: Administer medications, oxygen, feedings, and ostomy care according to the established plan of care. Monitoring & Reporting: Track vital statistics and promptly report any abnormal findings to the care team. Interdisciplinary Collaboration: Follow routines established by PT, OT, and Speech Therapists to ensure holistic recovery. Safety & Infection Control: Maintain strict infection control policies and report any communicable diseases. Clinical Documentation: Accurately record nursing actions, patient progress, and outcomes to ensure seamless communication with physicians and families. Logistics: Manage medical supply inventory and ensure all medical equipment is functional and maintained. HOME HEALTH LPN | REQUIREMENTS: Education: High School Diploma or GED (verifiable) Attendance: Commitment to mandatory attendance during the first 90 days. Licensure: Active, unrestricted LPN license in the State of Virginia. Must be able to work onsite and travel within the designated service counties. Preferred Skills & Experience: Wound care and Wound Vac experience. IV therapy and lab draws. Experience with Homecare Homebase (HCHB) software. Chronic disease management.
