AMERIPRIME HOME HEALTHCARE INC. looking to hire a Full Time Physical Therapists (PT) in the Central Valley area.Compensation: Salary non exempt - Depending on experienceAnnual Salary range $135,200.00-$141,440Additional perks:Flexible schedulingPaid sick timePTOEmployee benefitsEmployee referral programAreas of Responsibility:Makes initial admission/evaluation visitsIn consultation with the assigned Clinical Team Manager initiates and regularly re-evaluates and revises the plan of care. Instructs assigned PTA on the plan of care revisionsProvides accurate and timely documentation consistent with the plan of careCommunicates with the Clinical Team Manager or with the physician and other members of the home care team informing them of significant changes in the patients’ condition and needs. Documents these communicationsFollows established standards for point of service technology, documentation, and synchronizationSubmits all documentation according to agency requirements. Meets established productivity standardsPerforms other duties as assigned consistent with skills and trainingQualifications:Excellent assessment skills and excellent interpersonal skillsPositive attitude and ability to confront problems constructivelyhighly motivated, self-directed, flexible, and able to work well and participate in a multidisciplinary team settingRequirements:Graduate from an accredited physical therapy program approved by the Council on Medical Education of the American Medical Association in collaboration with the American Physical Therapy AssociationCurrent California PT LicenseMinimum one year home health experienceValid California driver's license and automobile insurance
