This role focuses on helping clients achieve stability and independence through comprehensive case management, advocacy, and coordination of services.Key Responsibilities:Conduct intake assessments and develop individualized service plansProvide ongoing case management and supportive counselingAssist clients with housing placement, benefits, and community resourcesMonitor client progress and update service plans as neededMaintain accurate and timely documentation in case management systemsAdvocate on behalf of clients with external providers and agenciesSupport clients in achieving self-sufficiency and long-term stabilityParticipate in team meetings and collaborate with multidisciplinary staffRequirements:Bachelor’s Degree required (Social Work, Psychology, Human Services, or related field)Minimum 1–2 years of case management experience, preferably in shelter or housing programsKnowledge of NYC housing systems, public benefits, and community resources preferredStrong interpersonal, organizational, and communication skillsAbility to work effectively in a fast-paced, high-need environment
