Our model focuses on ensuring those with end-stage renal disease receive the best possible care through a more convenient, centralized, continuous care model.Why is this Role Essential?The Hospital Liaison plays a critical role in supporting Concerto's market growth by serving as the primary in-market connection between hospitals, nephrologists, case managers, and our internal teams. This role ensures seamless patient referrals, strengthens provider relationships, and supports admission activities through accurate, timely documentation.The Hospital Liaison works hand-in-hand with the Market Development Manager and Market Development Associates to maintain strong referral pipelines and identify opportunities for expansion.You Might Be a Good Fit If...Relationship-oriented and approachableA proactive communicatorHighly organized and self-drivenAble to pivot quickly and manage changing prioritiesComfortable spending most of your time in the fieldMotivated by referral growth and collaboration What's Required?1-3 years of experience in hospital liaison work, healthcare sales, physician relations, or admissions/marketing within healthcareDialysis, SNF, hospital, or assisted living experience strongly preferredBachelor's degree preferred; equivalent professional experience acceptedStrong presentation and communication skillsProven ability to build and maintain relationships across hospitals, nephrology practices, and care facilitiesExperience gathering, organizing, and communicating clinical documentationProficiency with Salesforce, Microsoft Office, and other CRM or referral-tracking toolsStrong attention to detail with daily documentation habitsValid, unencumbered driver's licenseAbility and willingness to travel 100% in-market What Will You Do? 1. Referral Management & Follow-UpReview prior-week referral activity every Monday to identify gaps or changes in hospital behaviorsInvestigate lapses in referrals and re-engage key contactsFollow up on pending patients, missing documentation, and admission requirementsAdd documentation to admissions threads and ensure timely completion of required information 2. Daily In-Market Relationship Building (100% Travel) Spend Tuesday through Thursday traveling in-market visiting hospitals, nephrologists, case management teams, and other referral partnersMaintain a high-visibility presence across assigned hospitals and provider officesAdvocate for Concerto services and identify partnership opportunities 3. Hospital & Physician Presentations Conduct weekly hospital presentations to case management teams, social workers, and other stakeholdersDeliver educational materials clearly, professionally, and with consultative skill 4. Documentation & Salesforce Management Spend the first or last hour of each day completing Salesforce documentationAudit Salesforce entries for accuracy and completenessLog all activities, referrals, meetings, and outcomes in real time 5. Internal Collaboration Participate in weekly touchbases with the Market Development Manager (typically Monday or Friday)Communicate in-market observations, referral trends, and emerging opportunitiesSupport coordination with operations and admissions teams 6. Flexibility & Responsiveness Quickly pivot to urgent needs such as patient documentation issues, sudden referral delays, or unexpected hospital concernsAdapt to changing schedules, priorities, and market environments
