Your Role: As a Housing Case Manager, you will play a critical role in helping recently housed families and youth transition to long-term stability. Using a person-centered, wraparound approach, you’ll work closely with clients to create housing stabilization plans, conduct needs assessments, and connect them with vital community resources. You’ll also build and maintain strong relationships with landlords, property managers, and other community partners to remove barriers to housing and foster self-sufficiency. Key Responsibilities: Conduct comprehensive needs assessments to identify client strengths and barriers. Develop and implement housing stabilization plans tailored to each client’s unique needs. Collaborate with external resources and community partners to provide holistic support. Maintain strong, positive relationships with landlords, property managers, and community resource providers. Advocate for clients to ensure they receive the services and support needed for long-term stability. What We’re Looking For: Education: High School Diploma required. College degree (2 or 4-year) preferred, or an equivalent combination of education and experience. Experience: At least 3 years of direct experience in social services, case management, or a related field, ideally working with youth and families. Skills & Knowledge: Strong understanding of industry best practices and a willingness to complete or progress toward Community Health Worker (CHW) certification. Professional Qualities: Ability to collaborate effectively with clients, team members, and community partners, with a strong emphasis on professional communication, problem-solving, and maintaining healthy boundaries.
