Case Manager
```Duties``` - Provide case management services to individuals and families in need - Conduct assessments to determine client needs and develop individualized service plans - Coordinate and monitor services provided by various agencies and organizations - Advocate for clients' rights and access to resources - Collaborate with other professionals, such as social workers and healthcare providers, to ensure comprehensive care - Maintain accurate and up-to-date case records ```Skills``` - Childcare: Ability to provide support and guidance to families with children, including connecting them with appropriate childcare resources - Workforce development: Knowledge of employment services and programs to assist clients in finding and maintaining employment - Addiction counseling: Understanding of substance abuse disorders and ability to provide counseling and support to individuals struggling with addiction - Child welfare: Familiarity with child protection laws and regulations, as well as experience working with child welfare agencies - Medicare: Knowledge of Medicare programs and eligibility criteria, including the ability to assist clients in navigating the system - Crisis management: Ability to assess and respond effectively to crisis situations, providing immediate support and intervention - Mentoring: Experience in mentoring individuals, providing guidance, support, and encouragement - Crisis intervention: Skills in deescalating crisis situations, providing immediate assistance and support to individuals in distress - Behavioral health: Understanding of mental health disorders and ability to provide support and referrals for individuals in need of behavioral health services - Motivational interviewing: Proficiency in motivational interviewing techniques to engage clients in the change process Please note that this is not an exhaustive list of duties or skills required for the position.
