What You’ll Do
Provide full-scope primary care including preventive, acute, and chronic disease management
Lead and collaborate within a multidisciplinary care team
Perform exams, minor procedures, and prescribe per NYS guidelines
Coordinate care across services, including referrals, labs, and follow‑up
Engage in quality improvement and population health initiatives
Participate in community outreach and program development
What You Bring
MD or DO from an accredited medical school
Active New York State license and DEA
Board Certified or Board Eligible (within 18 months)
