Coverage Needed :Start Date: October 2026Schedule: Monday-Friday, 8am-4pm2 Clinic days, 3 OR days, Rotating weekend coverageFull timeCall: Alternating weekday and weekend callClinical Details :Inpatient and OutpatientEMR: EPIC (experience preferred, willing to train if needed)10-15 Clinic visits per day2-4 Surgical cases per OR daySupport Staff: 1 MA & 3 RNsRequirements :Active IN licenseActive IN DEA (or easily transferrable) and CSRBoard CertifiedCV must be in Month / Year formatMust have license in hand at time of submission, or proof that the license application has been submittedAny gaps greater than 30 days will require an explanation on the CVDEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
