LPN (Licensed Practical Nurse)

Overview We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including home care. As an LPN, you will play a crucial role in supporting patients with diverse needs, ensuring their comfort and well-being while adhering to established medical standards. Responsibilities Provide direct patient care, including administering medications and treatments as prescribed by physicians. Assist with IV insertion and perform gastric lavage when necessary. Conduct assessments and monitor patients' vital signs, reporting any changes to the supervising nurse or physician. Engage in dementia care, offering support and companionship to patients with cognitive impairments. Participate in discharge planning, ensuring that patients have the resources and support needed for a smooth transition from healthcare facilities. Collaborate with healthcare professionals to develop and implement individualized care plans. Perform triage duties as required, prioritizing patient needs effectively. Assist with medical imaging procedures, ensuring patient safety and comfort throughout the process. Stay updated on best practices and standards of care within the nursing field. Experience Current LPN licensure in the state of practice. Previous experience in home care or similar settings is preferred. Familiarity with botulinum toxin treatment is a plus. Strong understanding of nursing standards and protocols related to patient care. Excellent communication skills and the ability to work collaboratively within a healthcare team.
