Spiritual Care Counselor

I’m working with an established independent hospice facility in Davis, CA and we’re looking for a Spiritual Care Counselor to join us full-time (32-40 hours a week). You will provide for the spiritual needs of hospice patients, families, staff and volunteers.Essential Job Functions/Responsibilities: Spiritual Assessment and Consultation Ensures that patient/family spiritual needs are met and assists the Interdisciplinary Team (IDT) members in spiritual needs and issues. Provides consultation and education to interdisciplinary team members and others regarding spiritual issues and needs of patients, families and caregivers. Must meet visit expectations of 8-10 (eight to ten) visits per week. Contributes to the hospice Plan of Care (POC) for each patient. Officiates or assists in coordinating funeral services as requested by the patient/family. Responsible for organizing patient memorial services for staff and patient’s families. Assists Bereavement Services in the annual memorial service for staff and patient families. Interdisciplinary Team Communication Ensures continuity of care to patients/families by effectively collaborating and communicating with IDT members and the patient/family/caregivers. Attends IDT weekly meetings and actively participates in discussions related to patient care. Collaborates with IDT members regarding the patient/family plan of care to ensure continuity of care and the inclusion of spiritual issues. Documentation and Records Ensures compliance with the facility’s policies and procedures. Documents in the patient records for each contact with patient/family/caregivers. Contacts each patient/family admitted to the facility within 5 days of admission. Completes Spiritual Assessment for each patient within 5 working days of the admission. Qualifications: A minimum of four quarters of Clinical Pastoral Education training, or comparable experience is required. Knowledge of abuse/neglect report requirements affecting clinical practice required. Ability to communicate with people of all religious/spiritual backgrounds without enforcing personal views. Knowledge of the social and spiritual dimensions of terminal illness and of the Hospice concept.
