ECM/CS Program Coordinator
Access TLC is seeking an experienced and goal-oriented ECM/CS Program Coordinator to lead the day-to-day operations of our Enhanced Care Management (ECM) and Community Supports (CS) program.
As an ECM/CS Program Coordinator, you will supervise Lead Care Managers, oversee daily program operations, and ensure the program continues to meet performance, compliance, and quality standards across the Managed Care Plans we serve. You will play a central role in supporting the care management team, managing referrals and enrollment workflows, coordinating with Managed Care Plans, and helping drive continuous improvement across all aspects of the program.
This position is primarily based at our corporate office in Moorpark, with regular travel 1-2 days per week to our San Fernando office to support the team based there. Frequency may vary based on team needs. On-site presence is required to support daily supervision, team collaboration, in-person training, and program oversight activities. Candidates must be willing and able to commute between both office locations. Mileage for approved business travel will be reimbursed in accordance with company policy.

Compensation
$34.00/hour
Position Responsibilities
Supervise Lead Care Managers, providing guidance and performance oversight
Oversee day-to-day operations of the ECM and CS programs to ensure continued performance and compliance
Monitor caseload distribution, capacity, and workflow across the care management team
Manage referral, enrollment, and disenrollment processes across contracted Managed Care Plans
Monitor Community Supports referrals and delivery of authorized services
Coordinate with Managed Care Plan representatives on program requirements, reporting, and issue resolution
Support the implementation, updates, and adherence to program policies and procedures
Track program key performance indicators and prepare required reports and data submissions
Conduct quality assurance reviews of care management documentation, including MDT/ICT meeting notes, assessments, and care plans
Support audit preparation and respond to Managed Care Plan audit requests
Support onboarding and ongoing training of care management team members
May carry a small caseload of ECM members on an as-needed basis to support team capacity
Employees will be expected to travel to assigned locations as needed, and mileage will be reimbursed in accordance with company policy.
Requirements
Minimum 2 years of experience in Enhanced Care Management, Medi-Cal Managed Care, or a related care coordination program required
Familiarity with CalAIM and Managed Care Plan ECM/CS Provider Manuals required
Prior supervisory or team lead experience preferred
Bachelor's or Master's degree in one of the following preferred:

? Health or Human Services
Must have a California Driver's License, satisfactory driving record and a vehicle that is insured in accordance with state/or organization requirements.
Fluent in Spanish and English (preferred)
Strong organizational, communication, documentation, and leadership skills
Ability to manage multiple priorities and meet Managed Care Plan deadlines
Schedule
Full-Time Monday through Friday 8:30 a.m. – 5:00 p.m.
