**Job Title: Home Health Physical Therapist****Location:** Walnut Creek, CA**Job Description:**Join our team as a Home Health Physical Therapist, focusing on geriatric care. This 13• week contract, with potential extension, runs Monday to Friday, 8:00 am to 5:30 pm.**Responsibilities:**• Provide quality physical therapy to home health patients.• Manage a caseload of about 6 patients daily.• Schedule and coordinate patient visits.• Collaborate with RNs and OT/PT staff.• Use EMR systems like Oasis and HCHB (training available).**Requirements:**• Valid CA PT license.• BLS certification.• At least 1 year of home health experience.• Preferably experienced with geriatric patients.**Additional Information:**• No on• call or holiday duties.• Flexible scrub color.• Includes HR, HCHB training, and mentorship.
