Our senior leadership team is based locally in Avon, CT, ensuring responsive support and a strong connection to the communities we serve.Job Description The Social Work Designee (SSD) supports the psychosocial needs of residents under the direction of the Director of Social Services and in accordance with state and federal regulations. The Social Work Designee assists in implementing care plans, coordinating services, and ensuring residents and families receive appropriate support throughout their stay.Key Responsibilities:Assist in developing, implementing, and documenting care plans related to residents' social and emotional needs in the electronic medical recordSupport admission and discharge processes, including coordination with nursing, MDS, and rehabilitation teamsProvide support to residents and families regarding adjustment to the facility, care planning, and discharge needsEducate residents, families, and staff on residents' rights and available servicesMaintain and provide access to community resources and referral servicesAssist with applications and referrals (e.g., Medicaid, community services, guardianship/conservatorship)Address resident/family concerns and assist with resolution, ensuring appropriate documentation and follow-upMaintain regular communication with residents, families, and interdisciplinary team membersEnsure compliance with all state, federal, and facility policies and regulationsQualifications Associate's or Bachelor's degree in Social Work, Human Services, or related field preferredPrior experience in a long-term care setting preferredKnowledge of resident rights, admissions/discharge processes, and regulatory requirements preferredStrong organizational skills with attention to detail and ability to prioritize tasksExcellent interpersonal and communication skillsAbility to work collaboratively as part of an interdisciplinary teamCompassion, patience, and a positive attitude toward residents and familiesAbility to remain calm and professional in stressful situationsBenefits Apple Rehab offers a comprehensive benefits package for employees working 30+ hours per week, which may include:Scholarships and career growth opportunities4 Weeks Paid Time Off7 Paid HolidaysHealth Insurance BenefitsShort-Term & Long-Term DisabilityCall-a-Doc / 24/7 MD telephone serviceEmployee Assistance ProgramLife Insurance401(k) Retirement ProgramLongevity Credit
