Come be a part of a growing organization in a progressive, supportive, and innovative team!All levels of experience are encouraged to apply and will train the right new graduate.Job Type: Per DiemPay: $65-$80 per visitLocation: South Shore, Nassau CountySchedule: Flexible!What We Offer!1099 and W-2 options to fit your preferred work styleCompetitive salaryLongstanding SuccessOpportunities for professional growth and developmentA professional, supportive and fun work environment fostered by a team of well-respected clinicians who take pride in the challenges of meaningful workFocus on Patient Care - Quick EMR (No Oasis required)Great work/life balanceFlexibility and Autonomy - see patients any 7 days of the weekPreference given to see patients in your primary geographic areaResponsibilities:Accept and treat patients within your designated geographic areaProvide 1:1 treatmentEvaluate patients and develop individualized treatment plans based on physician orders and patient needsDeliver care that aligns with professional standards, agency policies, and the prescribed plan of careEducate family members, home health aides, and other caregivers on proper therapy techniquesPlan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.Provide timely and accurate documentation for each visitEvaluate effects of treatment at various stages and adjust treatments to achieve maximum benefitMust be willing to treat geriatric patients.QualificationsOccupational Therapist Program GraduateLicensed Occupational Therapist in the state of New York or eligible.Mandatory Certifications: First Aid, AED, & CPR Certifications.Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.Ability to travel to patients' homes within the assigned geographic territoryStrong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.Additional InformationThe anticipated pay range for this position is $65-80 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
