This role involves assessing, diagnosing, treating, and managing acute and chronic illnesses, promoting health and wellness, and working in collaboration with physicians and other healthcare providers. Conduct comprehensive physical exams and health assessmentsObtain and interpret patient histories and diagnostic testsDiagnose and treat common acute and chronic medical conditionsPrescribe medications and therapeutic treatments within the scope of practiceDevelop and implement individualized care plansProvide health education and counseling on disease prevention and healthy lifestyle choicesManage follow-up care and coordinate referrals to specialists when neededPerform minor procedures as appropriate (e.g., wound care, suturing)Document patient care accurately in the electronic health record systemParticipate in quality improvement and practice initiativesCollaborate effectively with physicians, nurses, and other healthcare team membersMaintain current knowledge of evidence-based practice guidelines.QualificationsMaster's or Doctorate degree from an accredited Nurse Practitioner program.If APRN was licensed prior to August 1, 1995, master's degree in nursing is preferred.Experience in primary care or family medicine preferredNP or PA Family MedicineNew grads will be reviewedExperienced APP preferredSchedule : M- F, call, w/e, holidays? Schedule is Monday- Thursday.
