They evaluate referrals for timely access to appointments, procedures and resources. They also collaborate with the health care team to revise patient care plans according to changing patient needs, to improve patient outcomes and satisfaction.They are responsible for:Coordinate and facilitate patient care across multiple specialties, ensuring timely scheduling of appointment, diagnostic tests, and procedures.Triage new patient referrals to ensure timely access to specialty care, prioritizing based on clinical need.Collaborate with multidisciplinary providers and staff to manage referrals, transition of care planning and follow up tracking and developing care plans.Conduct outreach to patients and/or caregivers to ensure their compliance with treatment plans, follow up visits, and post discharge needs.Identify and document barriers to care (medication access, transportation, insurance) and work with patients and providers to address them.Act as a liaison between patients, providers, staff, and referring providers to promote effective communication and coordination.About The University at BuffaloThe University at Buffalo (UB) #ubuffalo is one of America's leading public research universities and a flagship of the State University of New York system, recognized for our excellence and our impact.
