If you are interested in this Home Health LPN position, please contact Austin Faris at 586-710-7941 or afaris@alinestaffing.com Home Health LPN Compensation • The pay for this position is $37.30 per hour • Mileage reimbursement at $0.43 per mile • Benefits are available to full-time employees after 90 days of employment • A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates Home Health LPN Highlights • Contract to Hire opportunity • Onsite position covering Bergen County, NJ • Shift Requirements: Monday through Friday, 8:00 AM – 5:00 PM • Weekends and On-Call required • Weekly case conferences (Wednesdays at 8:45 AM for 1.5 hours) • Benchmark Requirement: Minimum 30 points per week (may vary based on patient/agency needs) • Timely documentation required within 24 hours Home Health LPN Responsibilities • Provide care for clients following the established plan of care while maintaining dignity and independence • Monitor vital signs and report abnormal findings • Administer medications, feedings, oxygen, ostomy care, and other treatments as prescribed • Perform skilled nursing services including wound care, wound vac management, Foley catheter care, and ostomy management • Complete routines established by PT, OT, and Speech Therapists • Maintain and enforce infection control policies and procedures • Accurately document all nursing actions, care provided, communication, progress, and outcomes • Order necessary supplies, medications, and feedings to ensure continuity of care • Monitor and report incidents, complete incident reports accurately and timely • Ensure medical equipment is properly maintained and report faulty equipment for repair Home Health LPN Requirements • Active Licensed Practical Nurse (LPN) license in the state of New Jersey • Skilled nursing experience (wound care, catheter management, ostomy care, etc.) • Experience with Home Care Home Base (HCHB) preferred • Comfortable using tablets, point-of-care technology, and Microsoft Teams • High School Diploma or GED • Attendance is mandatory for the first 90 days If you think this Home Health LPN position is a good fit for you, please reach out to me — feel free to call, e-mail, or apply to this posting!
