In this role, you will support patient care delivery, assist with care planning, and collaborate with the interdisciplinary team to ensure high-quality outcomes. This is a great opportunity for an LVN who enjoys working independently while being part of a supportive clinical team.QualificationsQualifications:Licensed Vocational Nurse (LVN) in the state of TexasMinimum of 1 year of home health experience preferredStrong knowledge of patient care, infection control, and documentation practicesAbility to follow and implement a Plan of Care (POC) under RN supervisionStrong clinical assessment, documentation, and communication skillsAbility to identify and report changes in patient conditionExperience working with interdisciplinary teams in a home health setting preferredReliable transportation with valid driver’s license and current auto liability insuranceAbility to read, write, and communicate effectively in EnglishEssential Functions:Provide skilled nursing care to patients in the home setting under the supervision of a Registered NurseAssist in identifying patient physical, psychosocial, and environmental needsParticipate in developing and implementing the Plan of Care (POC) in collaboration with the RNAdminister treatments and interventions in accordance with physician orders and agency policiesMonitor patient condition and report changes to the supervising RNMaintain accurate and timely clinical documentation in compliance with regulatory standardsParticipate in interdisciplinary communication and weekly case conferences (IDG/IDT)Provide verbal and written updates to the RN prior to reassessment, supervisory, and discharge visitsFollow and maintain infection control and safety protocols (OSHA standards)Ensure patient safety and quality of care at all timesComply with all state regulations and professional standards of nursing practiceParticipate in required in-service training and maintain clinical competencyMeet productivity expectations and ensure documentation is completed within required timeframesWe are an Equal opportunity employer; We offer an excellent benefit plan to include 401K with match, CEU reimbursement, vacation, sick, holidays, medical, dental, and supplemental insurance Plans as well as a Highly competitive compensation package.
