Required Skills, Experience, Education & Certifications:Required Education: Bachelor's Degree in Physical Therapy (PT)Required Years of Experience: minimum 1 year Required Licenses & Certifications: Certified Home Care experience (Part A) with OASIS knowledge and current usage. EPIC preferred. Registered Physical Therapist. BLS must be AHA. Note: We need a PT who already has certified (Medicare) home care experience and knows how to complete an OASIS assessment.
