IH+H currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.Job DescriptionThe Director of Healthcare Programs (RN) is a senior leadership role responsible for the strategic oversight, clinical quality, and operational integration of healthcare services across Illumination Health and Home programs. This role will lead all medical staff within the Recuperative Care program and supervise all Enhanced Care Management (ECM) programs and staff, while advancing and scaling care management services across the organization.As a key member of the senior leadership team, the Director will partner closely with operations to ensure high-quality, compliant, and patient-centered care delivery that aligns with organizational goals and community impact.The pay range for this position is $130,000 - $140,000 annually, depending on experience.ResponsibilitiesClinical Leadership & OversightProvide direct supervision and leadership to all medical staff within the Recuperative Care program, including RNs, LVNs, and medical assistantsEstablish and maintain clinical standards, protocols, and workflows to ensure high-quality, evidence-based careMonitor clinical performance metrics and implement quality improvement initiativesEnsure compliance with all regulatory, licensing, and accreditation requirementsProgram Development & Care ManagementLead the enhancement and expansion of the organization’s care management programs, including ECM (Enhanced Care Management) and related servicesDevelop scalable models that integrate medical, behavioral health, and social servicesIdentify opportunities for innovation, partnerships, and funding to support program growthUtilize data and outcomes to drive continuous improvement and demonstrate impactPartner closely with all CalAIM-related programs to ensure alignment, integration, and optimal performance across servicesOperational PartnershipCollaborate closely with the Operations team to align clinical services with program workflows, capacity planning, and client needsSupport efficient utilization of resources while maintaining high standards of careParticipate in program planning, budgeting, and performance managementLeadership & StrategyServe as a key member of the senior leadership team, contributing to organizational strategy, decision-making, and growth initiativesFoster a culture of collaboration, accountability, and mission-driven serviceRepresent Illumination Health and Home in external meetings with healthcare partners, payers, and community stakeholdersPreferred Experience/Minimum QualificationsRequired:Active Registered Nurse (RN) license in the State of CaliforniaBachelor’s degree in Nursing (BSN) required; Master’s degree in Nursing, Public Health, Healthcare Administration, or related field preferredMinimum of 5–7 years of progressive leadership experience in healthcare programsExperience managing clinical teams and overseeing care delivery in complex settingsStrong knowledge of care management models, including ECM and Medi-Cal populationsExperience working with underserved or vulnerable populationsPreferred:Experience in recuperative care, homeless services, or community-based healthcareFamiliarity with CalAIM initiatives and value-based care modelsProficiency in MS OfficeBenefitsMedical Insurance funded up to 91% by Illumination Foundation (Kaiser and Blue Shield), depending on the planDental and Vision InsuranceLife, AD&D and LTD Insurance funded 100% by Illumination FoundationEmployee Assistance ProgramProfessional Development Reimbursement401K with Company Matching15 days vacation PTO/year6 days sick PTO/year10 days holiday PTO/yearPotential eligibility for the Public Service Loan Forgiveness Program (PSFL) for federally qualified loansPowered by JazzHR2QEV5L2M6y
